Super Raffle Checklist

  • Please make checks and money orders payable to the NDPMA Parents Club.
  • Please fill out the ticket with the name and contact information of the purchaser.
  • Please return the portion of the ticket with the barcode and purchaser’s contact information to the Parents Club for inclusion in the raffle, while keeping the other half for your records.
  • Tickets can be mailed to the address given, deposited in the lock-box outside the chapel, or turned in during a communicated ticket drop event.  Please allow adequate time if mailing.
  • Please be aware of the incentive turn in date of Friday, September 27, 2019.  The community will benefit by achieving this goal.
  • For lost tickets or if you desire to sell additional tickets, please contact Leslie Czarnecki via email: czarnecki5@sbcglobal.net.
  • For question regarding tickets that have been submitted, please contact Kathy Conroy via email: kathyjoeconroy@comcast.net.
  • Please note that you must submit tickets and payment to be included in the raffle.  If you do not submit your families allotted amount of tickets, your account will be billed per the tuition agreement.
  • All tickets are due no later than Friday, September 27, 2019.
